Complaint - File a New Complaint

File a formal complaint (concerns about regulatory non-compliance) against a child care home, child care program, or a possible illegal operator.

 

If you have questions about what to file, please contact the Child Care Consumer/Licensor On Duty (LOD) Line at 1-800-649-2642, option 3.

 

Reporting Child Abuse and/or Neglect: The Child Care Consumer/LOD Line and the BFIS complaint form do not handle reports of child abuse or neglect. If you suspect that a child is being abused or neglected, call 1-800-649-5285 to report it - 24 hours a day, 7 days a week.

 

To file a complaint

  1. Click the Contact tab at the top of the page. Picture

 

  1. Click Complaint - File a New Complaint. Picture

 

  1. Enter the name of the individual or child care program of concern. Picture

 

  1. If available, enter the contact information of the individual or child care program. Picture

 

  1. In the Complaint Description box, enter a detailed description of the complaint, and then click Continue. Picture

 

  1. Choose one of the following, and then click Submit. Picture

 

    1. Provide your name and contact information, so we may contact you during the investigation and/or once an outcome has been determined.

 

    1. If you are submitting this request on behalf of someone else, please provide their contact information if they wish to provide it.

 

    1. If you wish to remain anonymous, do not enter any information.

 

  1. An Assigned Complaint No. displays when your complaint has successfully been submitted. Please keep this number so that you may view the status at a later date. Picture
