Complaint - File a New Complaint
This function is used to file a formal complaint (concerns about regulatory non-compliance) against a child care provider/program, or a possible illegal operator.
If you have questions about what to file you can contact the Child Care Consumer/Licensor On Duty (LOD) Line at 1-800-649-2642, option 3.
|Reporting Child Abuse and/or Neglect: The Child Care Consumer/LOD Line and the BFIS complaint form do not handle reports of child abuse or neglect. If you suspect that a child is being abused or neglected, call 1-800-649-5285 to report it - 24 hours a day, 7 days a week.|
To file a complaint in BFIS
- Click the
Contact tab at the top
of the page. Picture
Complaint - File a New Complaint.
Enter the name of the individual or program of concern. Picture
If available, enter the contact information of the individual or program. Picture
In the Complaint Description box, enter a detailed description of your complaint. When complete, click Continue. Picture
- Choose one of the following, and click
Submit when complete. Picture
- Provide your name and contact information, so we may contact you during the investigation and/or once an outcome has been determined.
- If you are submitting this request on behalf of someone else, please provide their contact information if they wish to provide it.
- If you wish to remain anonymous, do not enter any information.
- An Assigned Complaint
No. displays when your complaint has successfully been
submitted. Please keep this number so that you may view
the status at a later date. Picture
